Required Documents for Application Filing

To successfully file your application, ensure you bring the following:

1. ID Pictures (4 copies)

  • Specifications:
    • Philippine Passport size (4.5 cm x 3.5 cm or 1.8 in x 1.4 in).
    • Colored, with a white background.
    • Taken within the last three (3) months.
    • Printed on high-quality photo paper (no peeling or smudging).
    • Standard close-up shot, with the head and face occupying 80% of the picture.
    • Name tag handwritten (not computer-generated) with signature over printed full name:
      • Format: Given Name, Middle Initial (if any), Last Name, and Extension Name (if any).
  • Restrictions:
    • No eyeglasses or colored contact lenses.
    • No accessories that obstruct facial features.
    • Facial features must be clearly visible, including both ears.

2. Valid ID (Original and Photocopy)

Any of the following IDs are acceptable:

  • Driver’s License (temporary license must be accompanied by the old license and LTO OR).
  • Passport
  • PRC License
  • SSS, GSIS, or PhilHealth ID (must include photo, name, and signature).
  • Voter’s ID or Certification.
  • BIR ID (ATM/TIN card with picture).
  • Police or Barangay ID.
  • School or Company ID.
  • NBI Clearance or Seaman’s Book.
  • PWD, Solo Parent, Senior Citizen, or HDMF Transaction ID.
  • Philippine Identification (PhilID) Card.

3. Certificate of Consent

This form is available at the processing centers and must be duly completed.

4. Examination Fee

Applicants must pay ₱500 during filing.

5. Additional Requirements

  • For applicants without date of birth on their ID cards, provide the original and photocopy of a Birth Certificate issued by the Philippine Statistics Authority or Local Civil Registry.
  • Dual citizens or applicants recognized as Philippine citizens under R.A. 9225 must submit the original and photocopy of their Identification Certificate (IC) issued by the Bureau of Immigration.
